Release and Waiver of Liability
I hereby agree to release, waive and forever discharge Pilates BodyTree Pte Ltd,
BodyTree Academy Pte Ltd, their affiliates, and their respective employees,
agents, representatives and all independent contractor teachers from all claims,
demands, damages, costs, expenses, actions and causes of action, in respect to
death, injury, loss or damage to my person or property however caused directly
or indirectly, arising or to arise by reason of my practice or use of Pilates
BodyTree Pte Ltd, BodyTree Academy Pte Ltd facilities/equipment or
instructions (verbal and/or written), whether as a guest, client, spectator or
participant or otherwise.
In the training of members, Pilates BodyTree Pte Ltd and BodyTree Academy Pte Ltd have provided certain non-public, confidential and proprietary information of Pilates BodyTree Pte Ltd, BodyTree Academy Pte Ltd. Such information includes, but is not limited to, training material, training techniques, products, services and processes (“Confidential Information”). The Confidential Information is to be used by the member only in conjunction with his/her attendance and participation in the classes offered by Pilates BodyTree Pte Ltd, BodyTree Academy Pte Ltd. Under no circumstances is it to be used by the member to train others using the confidential information.
By registering here, you agree that Pilates BodyTree and BodyTree Academy Pte Ltd may collect, use and disclose your personal data, as provided in this application form for the following purposes in accordance with the Personal Data Protection Act 2012:
(a) the processing of this membership application; and
(b) the administration of the membership with our organisation.
By registering, I acknowledge I have read, understood and agree to the above Release and Waiver of Liability.
